Award and Fellowship Request Form 

Please complete this form to request an award or a fellowship for processing.

Take note of the following information:

  • One form submission can be made for multiple graduate students if the funding source and funding amount are the same for each student. 
  • If the fellowship will be paid via a disbursement schedule, please contact your RA prior to submitting the form for the schedule. 
  • For external fellowships, if there is an area that you are unsure of leave the field blank. If known, please be sure to include the RA's name and email.
